VO: This video provides an overview of 4D Molecular Therapeutics’ 4FRONT clinical studies and does not replace the informed consent form or discussions with your healthcare team. We encourage you to ask them any questions you may have.

VO: When you live with wet age-related macular degeneration—also known as wet AMD—everyday tasks that were once effortless, such as reading or driving, could become more difficult if left untreated.

VO: Though current wet AMD treatments potentially preserve vision, they require regular eye injections—as often as every 4 weeks—which can be uncomfortable and difficult to maintain.

VO: That’s why our 4FRONT clinical studies are evaluating 4D-150, an investigational gene therapy given as a potential one-time eye injection to treat wet AMD.

VO: Genes are segments of DNA and work like a blueprint and tell your

cells how to make specific proteins.

VO: Proteins allow the body to function properly.

VO: Gene therapy can be used to treat the underlying cause of a disease, by placing copies of a new gene into cells to allow the cells to produce protein that is needed to function properly.

VO: 4D-150 is an investigational gene therapy designed to help cells in the eye continuously produce commonly used medication to treat wet AMD.

VO: 4D-150 is intended to reduce treatment burden by minimizing the number of standard of care eye injections needed to maintain vision in people living with wet AMD. 

VO: It does not change your DNA, and the effects are limited to the treated eye.

VO: The Phase 1 and Phase 2 studies have shown 4D-150 to be well tolerated. Now it is being evaluated globally in Phase 3 clinical studies— 4FRONT-1 and 4FRONT-2—to test if it is effective and safe in comparison to the standard treatment for wet AMD.

VO: Your participation could help determine if 4D-150 is a treatment option for people living with wet AMD.

VO: Throughout the study, your safety and comfort are our highest priorities. You’ll receive support from your doctor and 4DMT to ensure you get the medical

care you need.

VO: Remember, you may leave the study at any time.

VO: To participate in either study, you must be at least 50 years old and diagnosed with wet AMD.

VO: The 4FRONT-1 study is being conducted in the US & Canada and is designed for people who have never been treated for wet AMD in their affected eye.

VO: And 4FRONT-2 is being conducted globally and is for people who have either never been treated for wet AMD or have been diagnosed within the past 6 months and have not received more than four injections of wet AMD treatment

in the affected eye.

VO: We understand travel for appointments can be difficult. If you participate, you may be reimbursed for distance driven or provided with private transportation to and from the study center.

VO: Additionally, you may receive compensation for completing study visits, in accordance with local guidelines.
